Driver Disclosure Declaration

Where the minibus will be used to transport children, young people, or vulnerable adults, the named driver(s) hold a valid PVG Scheme membership (Protecting Vulnerable Groups, Disclosure Scotland) or an equivalent enhanced DBS check, issued within the last 3 years.
Where no vulnerable passengers will be carried, the driver(s) hold at least a Basic Disclosure certificate or equivalent, issued within the last 3 years.
I understand that Alness Community Minibus reserves the right to request sight of disclosure documentation before confirming any booking.
I accept that providing false or misleading information in this declaration may result in cancellation of the booking and may be referred to the relevant authorities.
